Missional Forms is a subscription-based platform that allows churches to customize lead-generation forms, create gated content (e.g., Thanksgiving Activity Kits), and generate embed codes to place on their church websites. Visitors who fill out these forms will provide their names and email addresses, which Missional Forms collects and stores, making this data accessible to the respective church through their account.

3. Subscription and Payment

• Subscription Plans: Missional Forms offers monthly subscription plans. Payment is due at the beginning of each billing cycle.
• Non-Payment: Failure to make timely payments may result in immediate suspension or termination of your account without prior notice.
• No Refunds: All payments are non-refundable.

4. User Responsibilities and Content Restrictions

• Appropriate Use: Churches are solely responsible for the content they create and share via Missional Forms. All forms must be appropriate for church-related content and must not violate Biblical principles.
• Prohibited Content: Users may not create forms that are inflammatory, violent, promote disunity, unBiblical, or otherwise inappropriate. Missional Forms reserves the right to determine if content violates these standards.
• Right to Remove Content: Missional Forms reserves the right to remove any content and/or terminate a user’s access to the platform at any time, without notice, if we determine that the content violates these Terms of Use.

5. Data Collection and Privacy

• Data Collection: Missional Forms collects and stores the names and email addresses of individuals who complete forms embedded on church websites.
• Data Usage: This data is made available to the respective church through their account. Missional Forms does not sell or share this data with third parties.
• Data Security: While we take reasonable measures to protect user data, we cannot guarantee absolute security.

6. Termination of Service

Missional Forms reserves the right to terminate or suspend your access to the platform at any time, with or without cause, including but not limited to:

• Non-payment of subscription fees.
• Violation of content guidelines or misuse of the platform.

7. Limitation of Liability

Missional Forms is not responsible for any content created by users or the consequences of using such content. By using our platform, you agree to hold Missional Forms harmless from any claims, damages, or liabilities arising from the content you create or the data you collect.
